 for trauma and cardiac patients. The Cayuga LINK Transfer Center is available 24/7 as a centralized point of access for transfers. When you call 1-833-274-LINK, the complete transfer process will be handled using a well-coordinated approach to facilitate transfers appropriately and efficiently from Cayuga Medical Center and Schuyler Hospital.

Our goal is to streamline the patient transfer process by enhancing inter-facility communication and decreasing the amount of time spent by the referring medical community trying to coordinate a transfer from our hospitals.

How it Works:

One Call Emergency Transfer Center

The Cayuga LINK Transfer Center is the 
most effective way to arrange a patient transfer from Cayuga Medical Center and Schuyler Hospital.

By calling, the physician-to-physician communication time is drastically reduced to quickly provide patients with the quality of care they deserve.

To ensure a seamless patient transfer, the Transfer Coordinator will need the following information:

• Caller’s name
• Referring physician and hospital
• Contact number
• Service being requested
• Patient name
• Basic patient condition

How it works:

1. Call 1-833-274-LINK (1-833-274-5465)
2. The Cayuga LINK Transfer Center 
Coordinator will gather basic transfer information
3. The Coordinator will contact the specialist on call 
for the requested service
4. The Coordinator will facilitate a recorded 
conference call between the referring physician and the receiving facility’s on call physician to discuss the patient’s condition – the Coordinator will remain on the line as a third party
5. Once the patient is accepted for transfer, the coordinator will:
   1. Assist in making transportation arrangements
   2. Coordinate the nursing unit and bed 
arrangements needed for the patient
   3. Facilitate the transmission of necessary 
paperwork and medical documents
   4. Connect RN’s for patient report
   5. Once the patient has arrived at the receiving hospital facility, the Coordinator will call to confirm the patient’s arrival.
